Ozempic and Gallbladder Problems: Gallstones, Attacks, and When Surgery Is Needed

It is important to distinguish gallbladder attacks from the GI discomfort that GLP-1 medications commonly cause. GLP-1 nausea is diffuse, mild to moderate, and often constant or related to eating in general. Biliary colic is sudden, severe, localized to the RUQ, and specifically triggered by fatty meals. When It Becomes an Emergency

  • Gallstone pancreatitis — a gallstone obstructing the ampulla of Vater blocks both the bile duct and pancreatic duct, triggering acute pancreatitis. This combines the symptoms of biliary colic with the severe epigastric-to-back pain of pancreatitis. Gallstone pancreatitis is the most common cause of acute pancreatitis and can be life-threatening.
  • Gallbladder perforation — rare but catastrophic. A severely inflamed gallbladder can rupture, spilling bile into the peritoneal cavity and causing peritonitis. Signs include sudden worsening of pain followed by diffuse abdominal pain, rigidity, and sepsis.

How Common Are Gallbladder Problems on GLP-1 Medications?

Across the STEP clinical trial program for semaglutide 2.4mg (Wegovy), gallbladder-related adverse events (including cholelithiasis, cholecystitis, and biliary colic) occurred in approximately 1.6% of semaglutide-treated patients versus 0.7% on placebo over 68 weeks — a rate ratio of approximately 2.3. The decision to proceed to surgery depends on the clinical scenario. For recurrent biliary colic (multiple gallbladder attacks), elective cholecystectomy is recommended to prevent future attacks and potential complications. For acute cholecystitis, early cholecystectomy (within 24-72 hours) during the initial hospitalization is preferred over delayed surgery. For choledocholithiasis or gallstone pancreatitis, ERCP is typically performed first to clear the common bile duct, followed by cholecystectomy during the same hospitalization if the patient is stable.

An important consideration for GLP-1 users requiring surgery: anesthesiologists have raised concerns about the risk of pulmonary aspiration during anesthesia induction in patients on GLP-1 medications. Because these drugs slow gastric emptying, food and liquid may be retained in the stomach even after standard preoperative fasting (NPO after midnight).
